    Do you accept insurance?

    I am an out-of-network provider with all insurance companies, but I can provide a superbill, itemized receipt upon request. This can be submitted to your insurance company for direct reimbursement of your out-of-network benefits.

    You may choose to pay with cash, check, credit card or HSA card.

    Where are you located?

    Services can be accessed remotely via tele-health or in person visits can be arranged around the San Francisco peninsula  area. Additional fees may be charged for in person visits outside the San Francisco peninsula area. Please contact me for information.

    What do I need to bring?

     

    Please wear comfortable clothing that you can move freely in and allow access to specific area that needs to be treated. If you have a doctor referral, imaging documents, or any other pertinent information please have available.

    Do I need a doctors Referral for physical therapy?

    Under California law, patients can be seen directly for physical therapy services, and may continue to receive treatment for 12 visits or 45 days from initial evaluation, whichever occurs first. After which a physician referral/physician signed physical therapy plan of care is required for continued care.

    For wellness visits no prescription is necessary.
